Planning a care-management program

The decisions to make before choosing programs, devices, or partners.

Start with the population

Before choosing a program, describe the patients you want to support: their conditions, how often they are seen, and where gaps in between-visit care appear today.

Match programs to purpose

Each program has a different purpose. RPM adds structured home measurement; CCM and PCM maintain care plans; BHI coordinates behavioral health; TCM supports transitions; AWV organizes prevention.

Define responsibilities early

Agree who handles outreach, review, escalation, documentation, and billing before launch.

  • Who contacts patients, and when?
  • Who reviews information, and against which parameters?
  • How does a concern reach the provider?
  • Who documents, codes, and submits claims?

Decide how you will measure

Choose engagement, operational, and clinical measures before launch so results can be interpreted later.
