Care-management programs involve many hand-offs. Ambiguity about ownership is where delays, duplicated work, and documentation gaps arise.
Build a matrix
List each responsibility—identification, consent, outreach, review, escalation, documentation, coding, claims, reporting—and assign a single owner or a clearly defined shared arrangement.
Revisit after launch
Review the model after the first operating period and adjust as volumes and workflows mature.
